Home Visits

Diagnosing and treating medical problems is best done in our fully equipped surgery. The majority of medical conditions allow you to come to the surgery even in an emergency. However, if you are housebound or genuinely too ill to visit the surgery please contact the surgery as soon as possible after 8.00am in order to request a home visit. Please try to make the request before 10.00am if at all possible. A GP will call you back to discuss your request.

Outside surgery hours, you should ring
01784 420 700 and listen to the recorded message. Thames Doc provides an out of hours GP and home visit service.

New Patients

We ask new patients to complete a brief health questionnaire and offer a new patient check which includes measuring your height, weight, blood pressure and testing your urine.  This gives us some background information which will help in your future care.

Repeat Prescriptions

If you need medication regularly you should receive a repeat prescription form attached to your prescription.  We try to issue regular medicines for two or three months at a time and it saves work if you can order items together.  Please bring or send the form to us indicating what you need.  Postal requests need a SAE for posting back.  Prescription requests cannot be taken over the telephone.

Please fill in the online form if you wish to use our email repeat prescription service.  Your doctor will arrange to review your condition at appropriate intervals.

Registering online using GMS1 Form

You may register online using the GMS 1 form click here.

If you have recently registered with the practice please complete our online New Patient questionnaire click here.

To complete the formalities please bring into the practice 2 forms of ID
